Program Description: Twenty-one years ago, the Serious Hazards of Transfusion haemovigilance (SHOT) scheme began in the United Kingdom. It has had a significant impact on transfusion safety.< Dr. Bolton-Maggs has served as the Medical Director of SHOT from 2011 to 2018. She will review the background leading to its formation in 1996. In addition, she will describe how haemovigilance has made transfusions safer. However, errors are responsible for more than 80% of incidents and the SHOT scheme has made recommendations and contributed to national guidelines for transfusion practice which have reduced some of the most common errors. Dr. Bolton-Maggs will discuss the importance of the bedside checklist, to prevent wrong transfusions.
